- Initiate appropriate supportive treatment as dictated by the patient’s medical status.
- Closely monitor blood pressure and heart rate in these patients.
- Start at a lower starting dosage and increase the dosage every 4 to 6 weeks as needed based on clinical and laboratory response.
- Tell all your healthcare providers that you are taking Zepbound before you are scheduled to have surgery or other procedures.
Monitor for cardiac arrhythmias during surgical procedures in patients with coronary artery disease receiving suppressive SYNTHROID therapy. Monitor patients receiving concomitant SYNTHROID and sympathomimetic agents for signs and symptoms of coronary insufficiency. Administer SYNTHROID to pediatric patients who cannot swallow intact tablets by crushing the tablet, suspending the freshly crushed tablet in a small amount (5 to 10 mL) of water and immediately administering the suspension by spoon or dropper. Do not administer in foods that decrease absorption of SYNTHROID, such as soybean-based infant formula see Drug Interactions (7.9). Tell your doctor if you are pregnant or plan to become pregnant. Having hypothyroidism during pregnancy may increase the risk of premature birth or other complications.

Levothyroxine is given when your thyroid does not produce enough of this hormone on its own. Synthroid is a thyroid medicine that replaces a hormone normally produced by your thyroid gland to regulate the body’s energy and metabolism. Monitor serum free-T4 levels and maintain in the upper half of the normal range in these patients.
1 Pregnancy
For adult patients with primary hypothyroidism, titrate until the patient is clinically euthyroid and the serum TSH returns to normal. Exogenous thyroid hormone may produce regression of metastases from follicular and papillary carcinoma of the thyroid and is used as ancillary therapy of these conditions with radioactive iodine. Therefore, larger amounts of thyroid hormone than those used for replacement therapy are required. Medullary carcinoma of the thyroid is usually unresponsive to this therapy. Failure to respond to doses of 180 mg suggests lack of compliance or malabsorption.

TSH may not normalize in some patients due to in utero hypothyroidism causing a resetting of pituitary-thyroid feedback. HypothyroidismSYNTHROID® (levothyroxine sodium) tablets, for oral use is indicated as a replacement therapy in primary (thyroidal), secondary (pituitary), and tertiary (hypothalamic) congenital or acquired hypothyroidism. Assess compliance, dose of medication administered, and method of administration prior to increasing the dose of SYNTHROID. Biotin supplementation may interfere with immunoassays for TSH, T4, and T3, resulting in erroneous thyroid hormone test results. Inquire whether patients are taking biotin or biotin-containing supplements.
- Stop biotin and biotin-containing supplements for at least 2 days before assessing TSH and/or T4 levels see Drug Interactions (7.10).
- Concurrent use of sympathomimetics and SYNTHROID may increase the effects of sympathomimetics or thyroid hormone.
- Zepbound is also available in 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, or 15 mg per 0.5 mL doses in a single-dose pen (autoinjector).

Initiation of thyroid hormone therapy prior to initiating glucocorticoid therapy may precipitate an acute adrenal crisis in patients with adrenal insufficiency. Treat patients with adrenal insufficiency with replacement glucocorticoids prior to initiating treatment with SYNTHROID see Contraindications (4). Stop biotin and biotin-containing supplements for at least 2 days before assessing TSH and/or T4 levels see Drug Interactions (7.10). If so, advise them to stop biotin supplementation at least 2 days before assessing TSH and/or T4 levels see Dosage and Administration (2.4) and Drug Interactions (7.10). If you become pregnant while taking Synthroid, do not stop taking the medicine without your doctor’s advice.
